Zimacare Health Services offers:
- Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs in residential settings across Massachusetts.
- Critical Care Management (CCM), coordinating complex care for individuals with chronic conditions or medical needs.
- Home and Community-Based Services (HCBS), including personal care, home modifications, transportation, medication management, and supportive day programs.
All services are provided in alignment with applicable MassHealth care coordination, long term services and supports MA, and aging in place services Massachusetts standards.
- Eligibility and Participation
- Services are available to individuals who meet clinical and program-specific eligibility criteria.
- For MassHealth-funded programs, families must meet the applicable program requirements. Zimacare provides guidance but cannot guarantee eligibility.
- Participation requires consent from the individual or their legally authorized representative.
- Service Agreements
- All clients or their representatives will receive a detailed service agreement outlining care plans, responsibilities, and fees (if applicable).
- Agreements are subject to periodic review to ensure services continue to meet client needs and comply with Massachusetts regulations.
- Client Safety and Care Standards
- Zimacare ensures all staff are rigorously trained and maintain certifications necessary to deliver safe and professional care.
- Caregivers, nurses, and managers follow standardized protocols for medication management, emergency response, and Alzheimer’s or dementia support.
- Families are encouraged to maintain open communication with staff to ensure care remains responsive and individualized.
- Privacy and Confidentiality
- All personal, medical, and financial information shared with Zimacare is protected according to state and federal privacy laws, including HIPAA compliance.
- Data collected on this website or through service intake will only be used to deliver services or as required by law.
- Limitations and Disclaimers
